  1. Lake, river or other water body eligible for state or federal funding administered by FWC.
  2. Scientific name for the aquatic plant managed on that water body. Only one species is included on each form.
  3. The week in which management activities related to the identified plant were performed.
  4. All equipment used related to management activities for the identified plant. Government contractors apply for reimbursement following the FWC equipment use fee schedule.
  5. Time spent (hours) by the management crew (generally 1-2 persons) on management activities related to the identified plant species.
  6. Government contractors are eligible for overnight travel reimbursement for managing plants in remote locations or for attending training related to herbicide application certification.
  7. Expenses incurred by government contractors not included above that are eligible for reimbursement, including purchasing and cleaning uniforms and protective gear, publishing upcoming management events in local newspapers, etc.
  8. Time spent on various tasks related to managing the identified plant species. Government contractors are eligible for reimbursement for time spent on all activities.
  9. Time spent controlling the identified aquatic plant species.
  10. Time spent by government contractor traveling from the office to the boat ramp. Private contractor’s travel time is only eligible for reimbursement if the FWC Site Manager requests a meeting prior to the management activities.
  11. Time spent traveling by boat from the ramp to the control site on the waterbody. This expense is reimbursable for private and government contractors.
  12. FWC will reimburse up to three hours for time lost to unexpected weather conditions that arise after the contractor has commenced control operations.
  13. FWC will reimburse up to 1 hour for minor repairs that will keep the private or government contractor on-site increasing overall project cost-effectiveness rather than shutting down and returning to the contractor’s shop for repairs and losing a partial day’s work.
  14. List repairs that need to be done that caused a cessation of control operations. Time spent on Major Repairs is not eligible for FWC reimbursement.
  15. Includes activities related to cost-effective and responsible aquatic plant management such as meeting with the public, measuring wind speed, sampling water parameters including temperature, dissolved oxygen, and pH, collecting and mailing herbicide or plant samples, and posting signs at boat ramps related to the plant control activities. Also includes government contractor time spent filing aquatic plant management activity reports and invoices.
  16. FWC will reimburse a portion of government employee leave time.
  17. Time spent by contractors monitoring activities or conditions on the water body related to planning upcoming control events or evaluating recently completed aquatic plant control. Private contractors only participate and are eligible for reimbursement if their presence is requested by the FWC Site Manager.
  18. Time spent by the contractor pushing small floating islands or tussocks away from boat ramps or established navigation trails, or cutting and removing fallen trees or snags from established trails or river channels so management crews can get access to control aquatic plants.
  19. Time spent loading materials (i.e. herbicides) into the boat and mixing herbicides in the mixing tank. Also included is rinsing and preparing herbicide containers for proper disposal.
  20. The herbicide(s) and any adjuvant(s) applied to control the identified target plant.
  21. The contractor’s unit cost (per pound or gallon) to purchase herbicide and any adjuvant that were applied to control the identified target plant. If FWC supplies the herbicide or some other entity donates the herbicide(s), then a 0 is entered in this field.
  22. The amount of each herbicide and adjuvant used to control the identified target plant – regardless if purchased by the contractor or provided by FWC or other entity.
  23. Multiplies the unit cost per pound or gallon of herbicide and adjuvant times the total pounds or gallons applied during that week and sums the total cost for that week.
  24. The amount or dose of herbicide and any adjuvants that were applied – listed as pounds or gallons per acre, percent solution, or parts per million or billion.
  25. Occasionally more than one herbicide is applied in the tank mix to control the identified target plant to increase efficacy or selectivity, or as part of an herbicide resistance management strategy. If more than one herbicide is used, each herbicide is listed.
  26. The size of the area in which the target plant is intended to be controlled. Some systemic herbicide control strategies require more than one application, sometimes with as long as a month between successive applications in order to prolong the exposure period to control the target plant. In these cases, the acres controlled are listed for the first application but not for successive applications since the size of the intended control area remains the same.
  27. The sum of all acres of the identified target plant controlled during that week.
  28. A brief explanation is provided for each day that plant management activities were conducted describing the location where plants were controlled and any sampling or measurements taken including wind speed and dissolved oxygen content in the water.
  29. An explanation is required if the contractor has entered data for Lost Time Weather or Minor / Major Repairs. Detailed comments are also provided in this field if there is insufficient room in the Daily Activities field.
  30. Costs are summarized from the various reimbursement categories above. Government contractors are allowed up to 15% administrative cost derived by multiplying the percentage approved by FWC for the government contractor times any salary costs entered on the form.
  31. The signature of the contractor’s representative that conducted the work or filled out the form attesting that the work was completed and the expenditures were incurred.
  32. For government contractors, the signature of the contractor’s contract manager; for private contractors, the FWC representative for that contract area attesting that the aquatic plant management operation submitted on this form was conducted.
  33. The sum of all labor, equipment, and chemical cost plus any indirect cost that is allowed for a total cost for which the contractor is requesting reimbursement for the management of the identified plant on the water body for this particular week.
